Prevention

Doctors cannot always explain why one person gets cancer and another does not. However, scientists have studied general patterns of cancer in the population to learn what things around us and what things we do in our lives may increase our chance of developing cancer.

Anything that increases a person's chance of developing a disease is called a risk factor; anything that decreases a person's chance of developing a disease is called a protective factor. Some of the risk factors for cancer can be avoided, but many cannot. For example, although you can choose to quit smoking, you cannot choose which genes you have inherited from your parents. Both smoking and inheriting specific genes could be considered risk factors for certain kinds of cancer, but only smoking can be avoided. Prevention means avoiding the risk factors and increasing the protective factors that can be controlled so that the chance of developing cancer decreases.

Although many risk factors can be avoided, it is important to keep in mind that avoiding risk factors does not guarantee that you will not get cancer. Also, most people with a particular risk factor for cancer do not actually get the disease. Some people are more sensitive than others are to factors that can cause cancer. Talk to your doctor about methods of preventing cancer that might be effective for you.

Significance of oral cancer

Oral cancer may develop in any of 3 main areas:

  • Lips.
  • Oral cavity, which includes:
    • The front two thirds of the tongue.
    • The gingiva (gums).
    • The buccal mucosa (the lining of the inside of the cheeks).
    • The floor (bottom) of the mouth under the tongue.
    • The hard palate (the roof of the mouth).
    • The retromolar trigone (the small area behind the wisdom teeth).
  • Oropharynx, which includes:
    • The middle part of the pharynx (throat) behind the mouth.
    • The back one-third of the tongue.
    • The soft palate.
    • The side and back walls of the throat.
    • The tonsils.

Salivary glands are located throughout the oral cavity and oropharynx.

The most common type of oral cancer is squamous cell carcinoma, which begins in the thin, flat cells that line the oral cavity. Squamous cell carcinoma usually develops from lesions on the mucous membranes, including leukoplakia (white patches) and erythroplakia (red patches). Other types of oral cancer include lymphoma, sarcoma, melanoma, and salivary gland tumors.

Oral cancer occurs more often in men. The number of women with tongue cancer, however, has increased in recent years.

Oral cancer prevention

Oral cancer is sometimes associated with known risk factors for the disease. Many risk factors can be modified but not all can be avoided.

Tobacco and Alcohol Use: Tobacco use (cigarettes, pipes, cigars, and smokeless tobacco) is responsible for most cases of oral cancer. Alcohol, particularly beer and hard liquor, are associated with an increased risk of developing oral cancer. The risk of developing oral cancer is higher in people who use both tobacco and alcohol. Avoiding or stopping the use of tobacco decreases the risk of oral cancer. It is not known if stopping the use of alcohol decreases the risk of oral cancer.

Sun Exposure: Exposure to sunlight may increase the risk of lip cancer, which occurs most often on the lower lip. Avoiding the sun and/or using a sunscreen or colored lipstick on the lips may decrease the risk of lip cancer.

Other Factors: Some studies suggest that being infected with the human papillomavirus (HPV) may increase the risk of oral cancer.

Chemoprevention: Chemoprevention is the use of drugs, vitamins, or other agents to prevent or delay the growth of cancer or to keep it from coming back. Tobacco users who have had oral cancer often develop second cancers in the oral cavity or nearby areas, including the nose, throat, vocal cords, esophagus, and windpipe. Studies of chemoprevention in oral cancer are under way, including chemoprevention of leukoplakia and erythroplakia.

PDQ is the National Cancer Institute's (NCI's) comprehensive cancer information database. Most of the information contained in PDQ is available online at NCI's Web site 2. PDQ is provided as a service of the NCI. The NCI is part of the National Institutes of Health, the federal government's focal point for biomedical research.

PDQ contains cancer information summaries.

The PDQ database contains summaries of the latest published information on cancer prevention, detection, genetics, treatment, supportive care, and complementary and alternative medicine. Most summaries are available in two versions. The health professional versions provide detailed information written in technical language. The patient versions are written in easy-to-understand, nontechnical language. Both versions provide current and accurate cancer information.

The PDQ cancer information summaries are developed by cancer experts and reviewed regularly.

Editorial Boards made up of experts in oncology and related specialties are responsible for writing and maintaining the cancer information summaries. The summaries are reviewed regularly and changes are made as new information becomes available. The date on each summary ("Date Last Modified") indicates the time of the most recent change.

PDQ also contains information on clinical trials.

A clinical trial is a study to answer a scientific question, such as whether a certain drug or nutrient can prevent cancer. Trials are based on past studies and what has been learned in the laboratory. Each trial answers certain scientific questions in order to find new and better ways to help cancer patients and those who are at risk for cancer. During prevention clinical trials, information is collected about the effects of a new prevention method and how well it works. If a clinical trial shows that a new method is better than one currently being used, the new method may become "standard." People who are at high risk for a certain type of cancer may want to think about taking part in a clinical trial.

Listings of clinical trials are included in PDQ and are available online at NCI's Web site 5. Descriptions of the trials are available in health professional and patient versions. Glossary Terms

buccal mucosa (BUH-kul myoo-KOH-suh)
The inner lining of the cheeks.
cancer (KAN-ser)
A term for diseases in which abnormal cells divide without control. Cancer cells can invade nearby tissues and can spread to other parts of the body through the blood and lymph systems. There are several main types of cancer. Carcinoma is cancer that begins in the skin or in tissues that line or cover internal organs. Sarcoma is cancer that begins in bone, cartilage, fat, muscle, blood vessels, or other connective or supportive tissue. Leukemia is cancer that starts in blood-forming tissue such as the bone marrow, and causes large numbers of abnormal blood cells to be produced and enter the blood. Lymphoma and multiple myeloma are cancers that begin in the cells of the immune system. Central nervous system cancers are cancers that begin in the tissues of the brain and spinal cord.
cell (sel)
The individual unit that makes up the tissues of the body. All living things are made up of one or more cells.
chemoprevention (KEE-mo-pre-VEN-shun)
The use of drugs, vitamins, or other agents to try to reduce the risk of, or delay the development or recurrence of, cancer.
clinical study
A type of research study that tests how well new medical approaches work in people. These studies test new methods of screening, prevention, diagnosis, or treatment of a disease. Also called a clinical trial.
drug
Any substance, other than food, that is used to prevent, diagnose, treat or relieve symptoms of a disease or abnormal condition. Also refers to a substance that alters mood or body function, or that can be habit-forming or addictive, especially a narcotic.
erythroplakia (eh-RITH-roh-PLAY-kee-uh)
An abnormal patch of red tissue that forms on mucous membranes in the mouth and may become cancerous. Tobacco (smoking and chewing) and alcohol may increase the risk of erythroplakia.
esophagus (ee-SAH-fuh-gus)
The muscular tube through which food passes from the throat to the stomach.
gene
The functional and physical unit of heredity passed from parent to offspring. Genes are pieces of DNA, and most genes contain the information for making a specific protein.
gingiva (JIN-jih-vuh)
The tissue of the upper and lower jaws that surrounds the base of the teeth. Also called gums.
hard palate (PAL-et)
The front, bony part of the roof of the mouth.
human papillomavirus (HYOO-mun PA-pih-LOH-muh-VY-rus)
A member of a family of viruses that can cause abnormal tissue growth (for example, genital warts) and other changes to cells. Infection with certain types of human papillomavirus increases the risk of developing cervical cancer. Also called HPV.
infection
Invasion and multiplication of germs in the body. Infections can occur in any part of the body and can spread throughout the body. The germs may be bacteria, viruses, yeast, or fungi. They can cause a fever and other problems, depending on where the infection occurs. When the body’s natural defense system is strong, it can often fight the germs and prevent infection. Some cancer treatments can weaken the natural defense system.
inherited (in-HAYR-it-ed)
Transmitted through genes that have been passed from parents to their offspring (children).
lesion (LEE-zhun)
An area of abnormal tissue. A lesion may be benign (noncancerous) or malignant (cancerous).
leukoplakia (LOO-koh-PLAY-kee-uh)
An abnormal patch of white tissue that forms on mucous membranes in the mouth and other areas of the body. It may become cancerous. Tobacco (smoking and chewing) and alcohol may increase the risk of leukoplakia in the mouth.
lymphoma (lim-FOH-muh)
Cancer that begins in cells of the immune system. There are two basic categories of lymphomas. One kind is Hodgkin lymphoma, which is marked by the presence of a type of cell called the Reed-Sternberg cell. The other category is non-Hodgkin lymphomas, which includes a large, diverse group of cancers of immune system cells. Non-Hodgkin lymphomas can be further divided into cancers that have an indolent (slow-growing) course and those that have an aggressive (fast-growing) course. These subtypes behave and respond to treatment differently. Both Hodgkin and non-Hodgkin lymphomas can occur in children and adults, and prognosis and treatment depend on the stage and the type of cancer.
melanoma (MEH-luh-NOH-muh)
A form of cancer that begins in melanocytes (cells that make the pigment melanin). It may begin in a mole (skin melanoma), but can also begin in other pigmented tissues, such as in the eye or in the intestines.
mucous membrane (MYOO-kus...)
The moist, inner lining of some organs and body cavities (such as the nose, mouth, lungs, and stomach). Glands in the mucous membrane make mucus (a thick, slippery fluid). Also called mucosa.
oral cancer (OR-ul KAN-ser)
Cancer that forms in tissues of the lip or mouth. This includes the front two thirds of the tongue, the upper and lower gums, the lining inside the cheeks and lips, the bottom of the mouth under the tongue, the bony top of the mouth, and the small area behind the wisdom teeth.
oral cavity (OR-ul KA-vuh-tee)
The mouth.
oropharynx (or-o-FAIR-inks)
The part of the throat at the back of the mouth. It includes the soft palate, the base of the tongue, and the tonsils.

pharynx (FAIR-inks)
The hollow tube inside the neck that starts behind the nose and ends at the top of the trachea (windpipe) and esophagus (the tube that goes to the stomach). The pharynx is about 5 inches long, depending on body size. Also called the throat.
preventive
Used to prevent disease.
retromolar trigone (reh-troh-MOH-ler TRY-gone)
The small area behind the wisdom teeth.
risk factor
Something that may increase the chance of developing a disease. Some examples of risk factors for cancer include age, a family history of certain cancers, use of tobacco products, certain eating habits, obesity, lack of exercise, exposure to radiation or other cancer-causing agents, and certain genetic changes.
salivary gland (SA-lih-VAYR-ee gland)
A gland in the mouth that produces saliva.
salivary gland cancer (SA-lih-VAYR-ee gland KAN-ser)
A rare cancer that forms in tissues of a salivary gland (gland in the mouth that makes saliva). Most salivary gland cancers occur in older people.
sarcoma
A cancer of the bone, cartilage, fat, muscle, blood vessels, or other connective or supportive tissue.
second primary cancer
Refers to a new primary cancer in a person with a history of cancer.
soft palate (PAL-et)
The back, muscular (not bony) part of the roof of the mouth.
squamous cell carcinoma (SKWAY-mus sel KAR-sih-NOH-muh)
Cancer that begins in squamous cells, which are thin, flat cells that look like fish scales. Squamous cells are found in the tissue that forms the surface of the skin, the lining of the hollow organs of the body, and the passages of the respiratory and digestive tracts. Also called epidermoid carcinoma.
sunscreen
A substance that helps protect the skin from the sun's harmful rays. Sunscreens reflect, absorb, and scatter both ultraviolet A and B radiation to provide protection against both types of radiation. Using lotions, creams, or gels that contain sunscreens can help protect the skin from premature aging and damage that may lead to skin cancer.
tongue cancer
Cancer that begins in the tongue. When the cancer begins in the front two-thirds of the tongue, it is considered to be a type of oral cavity cancer; when the cancer begins in the back third of the tongue, it is considered to be a type of oropharyngeal or throat cancer.
tonsil
One of two small masses of lymphoid tissue on either side of the throat.
vitamin
A key nutrient that the body needs in small amounts to grow and stay strong. Examples are vitamins A, C, and E.
vocal cord (VOH-kul kord)
One of two small bands of muscle within the larynx that vibrates to produce the voice.
windpipe
The airway that leads from the larynx (voice box) to the bronchi (large airways that lead to the lungs). Also called trachea.
