Added text about systematic changes to the histological interpretation of biopsy specimens during the prostate-specific antigen screening era with the subsequent tendency by pathologists to read and treat high-grade tumors more aggressively (cited Albertsen et al. as reference 11 and Thompson et al. as reference 12).
